And/or upgrade PHPBB, and add some form of captcha thing on the registration form.

I'll put in a vote for vBulletin. You can migrate/import from phpBB and it has a slew of features that phpBB doesn't have including more rigorous/flexible anti-spam features. It isn't that expensive. We switched to it for our regional (SCCA) forums years ago and never looked back.
